Proposal form
Also called application form.
The application document. Its answers form the basis of the contract.
The proposal form is where you state the facts the insurer relies on. Different products attract different forms, and the medical depth varies sharply between them — annuity forms carry no health questionnaire at all, while term cover attracts the fullest.
The answers become part of the contract, which is why accuracy matters more than speed.
